Hospice is a patient and family oriented healthcare program that emphasizes increasing the quality of life. Dr. Cicely Saunders developed the first modern hospice by listening to patients’ end of life stories and developed a concept, which focuses on every aspect of pain management towards the end of life including physical, social, emotional, and spiritual pain. By addressing patients’ and their family’s needs in these four core areas during the final stages of life, Integrity Home Health & Hospice is able to attain its goal of constantly controlling the pain associated with these four core areas.

Hospice initially was a program completely funded by volunteer work. In 1983, the hospice program was introduced and recognized originally as a Medicare benefit, but now most insurance programs have a hospice benefit that can be used to fund Integrity’s services. Volunteer work remains a key focus of our hospice program as we maintain a loving and qualified team of volunteers that give their time freely to assist patients and their families in the final stages of life.

By recognizing and managing the four core areas of pain towards the end of life, hospice care benefits from a team approach. Integrity Home Health & Hospice provides an array of services to our patients through our interdisciplinary team including physician, nursing, social, and chaplain services. Under the Medicare hospice benefit, Integrity Home Health & Hospice patients have access to the following services:

• Attending Physician – Integrity works directly with your current physician to provide you with the services you need.
• Hospice medical director – Integrity has a 24/7 on-call physician available for times when your primary physician is not available.

Medications

• Integrity provides all medications needed for the alleviation and palliation of terminal illness.

Nursing Services

• 24/7 on-call Registered Nurse (RN) services
• Certified Nursing Assistant (CNA) services
• Home-making services
• Personal care services and assistance with activities of daily living

• Social Work Counseling
• Bereavement Services to help patients’ and their families deal with grief. An individualized grief support plan of care is implemented and continues for 13 month after the death of a hospice patient.
